Fix setup of `pylaprof-merge`.

Update author email.

Improve implementation and documentation of `PYLAPROF_DISABLE`.

Relax parsing of the `PYLAPROF_DISABLE` env. variable: in case of invalid value
just fall back to `true` and do not raise an exception.

First production-ready release 🎉
