- Removed the hard-coded version number in `setup.py` so the git-based version will be used.

- Exported the plugin as a `pyocd.probe` entry point. This makes it possible for pyocd to find pyocd_pemicro once the package is installed.
- Ensured the package can be released as a universal wheel.
