Sphinx-mdolab-theme

This is a minor version that adds an optional parameter `type`, which defaults to `options` but can be set to `informs` to print informs tables.

This is a minor release that adds a default logo to the docs.

This is a patch release, mainly to test the new Azure deployment system. There are no content changes for this package.

This is the first official release of the theme. After several months of testing this is now finally mature enough to come out of pre-release.
Only minor changes compared with the previous v0.x release:
- sphinx >= 3.0 is now required
- the copyright year has been updated

Added `sphinx>=3.0` requirement.

Added better way to incorporate CSS files.
